Care Management Services

The Alliance provides complex case management y health education and disease management services. Refer SPDs to these programs as appropriate.

Transportation Assistance

Call 800-700-3874, ext. 5577.

• For non-emergency medical transportation (NEMT): please call at least 5 days prior to an appointment.
• For Non-medical Transportation (NMT): please call at least 7 days prior to an appointment.

Language Assistance Services

The Alliance offers interpreter services for eligible Alliance members with limited English proficiency (LEP) or who are deaf or hard of hearing. All services are free of charge for providers and members.

Alliance Provider Bulletin

The Cultural Crossroads section of the Alliance Provider Bulletin provides information on communicating with SPDs and culturally diverse populations.
