Join a network of providers dedicated to pursuing the Alliance vision of Healthy People, Healthy Communities. To become a provider in the Alliance network, you must go through a three-step credentialing process. Medi-Cal Certification and Enrollment
All providers contracted with the Alliance are required to complete the Department of Health Care Services (DHCS) Medi-Cal screening and enrollment process. To enroll as a Medi-Cal provider with DHCS, visit the Provider Enrollment Division page on the DHCS website.

Become an Approved Alliance Provider
Application Request Form
Complete an online Provider Interest Form or call 831-430-5504 to notify our Provider Relations department of your interest.
Provider Relations will explain which credentialing application, forms and supplemental documents you must submit. They will also send you a Services Agreement to sign and return.
If you are a Medi-Cal primary care physician, we will perform a site review that must result in an acceptable score before any members are linked to your practice.
An Alliance medical director or the Peer Review and Credentialing Committee (PRCC) must approve your credentialing.
Once you are approved, we will send you an approval letter and countersigned Services Agreement. To get started working with the Alliance, view our Provider Orientation resources.
