How to Join
To become a Medi-Cal provider in the Alliance network, you will need to:
- Be screened and enrolled as a Medi-Cal provider with DHCS.
- Be approved by the Alliance as a provider. This process consists of:
- An application.
- A site review.
- Credentialing approval.
If you would like to be a TotalCare (HMO D-SNP) provider, you will need to be enrolled as both a Medi-Cal and Medicare provider. You can find more information on becoming a Medicare provider on the CMS website.

Medi-Cal certification and enrollment
- Get screened and enrolled as a Medi-Cal provider with DHCS.
All providers contracted with the Alliance are required to complete the Department of Health Care Services (DHCS) Medi-Cal screening and enrollment process.
To enroll as a Medi-Cal provider with DHCS, visit the Provider Enrollment Division page on the DHCS website.

Become an approved Alliance provider
To become an approved Alliance Provider, you must submit a completed application, pass a site review and have your credentialing approved by an Alliance medical director or the Peer Review and Credentialing Committee.
- Complete the appropriate application and sign the Services Agreement. Credentialing applications and policies are available on our website. Please check with the Provider Network team to confirm which application to complete.

- Pass the site review.
If you are a Medi-Cal primary care physician, we will perform a site review that must result in an acceptable score before any members are linked to your practice.
- Get your credentialing approved. An Alliance medical director or the Peer Review and Credentialing Committee (PRCC) must approve your credentialing.
After you are approved
Once you are approved, we will send you an approval letter and countersigned Services Agreement.
