Join a network of providers dedicated to pursuing the Alliance vision of Healthy People, Healthy Communities. To become a provider in the Alliance network, you must go through a three-step credentialing process. If you have any questions, please call 831-430-5540 and a Provider Relations Representative will assist you.
Medi-Cal Certification and Enrollment
All providers contracted with the Alliance are required to complete the Department of Health Care Services (DHCS) Medi-Cal screening and enrollment process. To enroll as a Medi-Cal provider with DHCS, visit the Provider Enrollment Division page on the DHCS website.
You can also contact DHCS Provider Enrollment Division at PEDCorr@dhcs.ca.gov or 916-323-1945.
Become an Approved Alliance Provider
Step One: Application Request Form
Complete an online Application Request form or call 831-430-5540 to notify our Provider Relations department of your interest.
Provider Relations will explain which credentialing application, forms and supplemental documents you must submit. They will also send you a Services Agreement to sign and return.
Step Two: Site Review
If you are a Medi-Cal primary care physician, we will perform a site review that must result in an acceptable score before any members are linked to your practice.
Step Three: Credentialing Approval
An Alliance medical director or the Peer Review and Credentialing Committee (PRCC) must approve your credentialing.
The PRCC meets quarterly, so to be considered for credentialing, you must submit your application documents before the following dates:
- September 8, 2021.
- December 8, 2021.
Once you are approved, we will send you an approval letter and countersigned Services Agreement. To get started working with the Alliance, view our Provider Orientation resources.
CCS Provider Paneling
If you want to become California Children’s Services (CCS) paneled, fill out the online application on the DHCS CCS provider paneling website.
