About TotalCare (HMO D-SNP) Primary Care services
Primary care, also called routine care or regular health care, includes preventive care to help you stay healthy and get medical help when you are sick. As a TotalCare (HMO D-SNP) plan member, you can get primary care by visiting a qualified primary care provider (PCP) who is part of this plan. A list of PCPs is included in the TotalCare Provider Directory. There is no copay when you see your PCP.
You can start getting health care services as soon as your coverage begins. You will receive a letter from TotalCare letting you know that Medicare approved your enrollment and when your coverage begins. Remember to always carry your TotalCare ID card and Medi-Cal Benefits Identification Card (BIC) with you whenever you get health care services. In most cases, you will only need your TotalCare card to get Medicare and Medi-Cal services. Do not let anyone else use your TotalCare ID card or BIC.
If you are new to the Alliance or to TotalCare, you will need to pick a primary care provider (PCP). You will see your PCP for most of your health care needs. You can use our online, searchable Provider Directory, read through the Provider/Pharmacy Directory that you can download, or you can call Member Services, and we can help you find the right doctor.
You must pick a PCP in the TotalCare network at the time of enrollment. If you do not, we will assign you a PCP. You can change your PCP anytime.
