Providers can now submit Lead Screening in Children and Post-Discharge Care measures through the Data Submission Tool (DST).
- Lead Screening in Children: The percentage of children 2 years of age who had one or more capillary or venous blood tests for lead poisoning by their second birthday. Review the Lead Screening Quality Report on the Provider Portal in comparison to your Electronic Health Record (EHR). Missing Lead Screening information can now be submitted via the DST, including screenings completed by other clinic sites.
- Post-Discharge Care: Members who receive a post-discharge visit within 14 days of discharge from a hospital inpatient stay. This measure pertains to acute hospital discharges only. Emergency room visits do not qualify. The DST accepts post-discharge visits completed by a linked primary care provider (PCP) or specialist.
