2025 Community Impact Report

The Alliance has released its 2025 Community Impact Report, highlighting investments that improved health equity in 2024. The report highlights the progress we have made in addressing social drivers of health and expanding access to care for our members, which wouldn’t be possible without the extensive collaboration from our provider network.

Key highlights include:

  • $93 million in community investments supporting housing, workforce development and education.
  • $30 million for housing, creating 824 permanent supportive units and 210 temporary beds.
  • Expanding the Community Health Worker (CHW) and doula networks with 38 CHWs and 12 doulas added in 2024.
  • $15 million for case management and community support addressing homelessness and social health needs.
  • $16 million for healthcare facilities, improving access for over 72,000 Medi-Cal members.

The Alliance’s Medi-Cal Capacity Grant Program (MCGP) invests in health care organizations to improve coordination across the health care system and address gaps in care for Medi-Cal members in Mariposa, Merced, Monterey, San Benito and Santa Cruz counties.

MCGP grants include:

  • Workforce Recruitment. These grants fund provider organizations to recruit and hire personnel to serve the Medi-Cal population. This includes community health workers, doulas, medical assistants and specific kinds of licensed health care professionals in Alliance service areas. The Alliance also offers a Linguistic Competence Provider Incentive for hiring bilingual providers.
  • Healthcare Technology. These grants support the purchase and implementation of infrastructure that improves access, convenience and care coordination for Medi-Cal members.
  • Data Sharing Support. These grants provide funding for infrastructure, operational solutions and technical assistance to build capacity to meet Medi-Cal requirements by sharing real-time health care data and connecting to a heath information exchange (HIE).
