The Complex Case Management team partners with PCPs to provide support to members with complex medical needs. The Care Coordination team assists members with less complex and non-clinical needs.

Every TotalCare (HMO D-SNP) member is assigned a care manager when they enroll. TotalCare care management focuses on identifying and addressing members’ medical, behavioral health, functional and social needs, coordinating across care teams and community resources to help members achieve their personal health goals.
