Grants to support the construction, renovation and/or acquisition of medical facilities, mobile medical clinics and community-based and school-based wellness centers that will serve the Medi-Cal population in the Alliance service areas.
Grant to support two-year pilot to build on existing children’s savings account (CSA) program in Santa Cruz County for eligible Alliance members 0-2 years old to achieve two preventative care milestones for immunizations and well-child visits.
Grants to support the hiring of locum tenens providers and/or to allocate additional hours for existing staff to augment clinic hours and/or days to assist with closing the remaining Managed Care Accountability Sets (MCAS) care gaps. Grant funds also support purchase and implementation of medical equipment or supplies that directly relate to the quality measures targeted for improvement.
Grants to support the development of new educational programs for health care professionals that will serve the Medi-Cal population.

Grants to community-based organizations to help Medi-Cal members impacted by COVID-19 to receive supplies for basic needs such as food, hygiene and sanitation supplies.
Grants to support Medi-Cal providers for infrastructure, operational solutions and technical assistance to build capacity to meet Medi-Cal requirements by sharing real-time health care data and connecting to a heath information exchange (HIE).
Grants to subsidize the cost of equipment that expands providers’ capacity to serve additional Medi-Cal members, improve practice efficiencies and improve member experience.
Grants to increase the number of trained providers to improve access to equity-oriented health care for the Medi-Cal population in the Alliance service areas.
Grants to support maternal, infant and child health in the first five years of life and remove barriers to preventative health care for the Medi-Cal population in the Alliance service areas.

Grants to support high-volume primary care practices in adding staff to provide intensive case management services for medically complex Medi-Cal patients within the patient-centered medical home (PCMH).
Grants to increase access to opportunities for community-based physical activity for the Medi-Cal population in the Alliance service areas.
Grants to support community-based projects that align with a food prescription model, including medically supportive food distribution coupled with an education and/or skill-building intervention, to improve member health and nutritious food security.
Grants to fund the delivery of ready-made, nutritious meals to Medi-Cal members recovering from an inpatient hospital stay.
Grants to support practices in changing their clinic operations to improve quality, efficiency and patient experience through the adoption of the PCMH model of care.
Grants to recuperative care facilities to provide temporary housing with access to medical and other supportive services for Alliance Medi-Cal members experiencing homelessness and who are recovering from an acute illness or injury.
Grants for practice coaching or other consultant/training opportunities.
