Health Education and Disease Management
The Alliance is committed to improving access to affordable health care for our members.
Alliance health education and disease management programs are managed by a multidisciplinary team of experienced health educators. Alliance health educators utilize motivational interviewing and trauma-informed care techniques to help members stay healthy and manage chronic disease.
Health program interventions follow evidence-based models/curriculums and/or a combination of best practices. Interventions can be done telephonically and via in-person workshops, virtual learning and member mailings.
Member Health Rewards Program
Alliance members can earn incentives for getting routine care and managing chronic conditions through our Health Rewards Program.
To refer an Alliance member to one of our programs, please complete the Health Education and Disease Management Programs referral form and fax it to 831-430-5852, attention: Health Programs. Do not include visit notes or lab results.

Interpreter Services
The Alliance offers interpreter services for eligible Alliance members with limited English proficiency (LEP) or who are deaf or hard of hearing. All services are free of charge to providers and members.
