Health Education and Disease Management
The Alliance is committed to improving access to affordable health care for our members. Health education functions as part of our care management services, which help members and their caregivers maintain and improve health and quality of life.
Alliance health education and disease management programs are managed by a multidisciplinary team of experienced health educators. Alliance health educators utilize motivational interviewing and trauma-informed care techniques to help members stay healthy and manage chronic disease.
Health program interventions follow evidence-based models/curriculums and/or a combination of best practices. Interventions can be done telephonically and via in-person workshops, virtual learning and member mailings.
Health and Wellness Rewards Program
Members can receive rewards for their participation in health education and disease management programs through the Alliance’s Health and Wellness Rewards Program. Only Alliance Medi-Cal members who do not have other health insurance are eligible to participate in the rewards program.
Health Programs staff work with members to refer them to health education classes, provide them with health education materials and refer them to community resources and services that are culturally and linguistically appropriate for the Alliance’s diverse membership.
To refer an Alliance member to one of our programs, please complete the Health Education and Disease Management Programs referral form and fax it to 831-430-5852, attention: Health Programs. Do not include visit notes or lab results.

Medi-Cal eligibility is established monthly, and providers should confirm eligibility at the time of service. To confirm patient eligibility, call the Automated Eligibility Verification Line at 831-430-5501 (24 hours).
Please note that each referral is assessed for program eligibility and requirements.

The Alliance offers interpreter services for eligible Alliance members with limited English proficiency (LEP) or who are deaf or hard of hearing. All services are free of charge to providers and members.
