Introducing Enhanced Care Management and Community Supports
Care coordination, housing assistance and other services for eligible members
Starting in 2022, the Alliance will offer services to eligible members through two programs called Enhanced Care Management and Community Supports. These services will help people with complex health issues and people who need help setting up their medical care. They can help you get healthy and stay as healthy as possible.
Enhanced Care Management (ECM) can help you set up medical care and get community services. ECM services start January 1, 2022 in Santa Cruz and Monterey counties. In Merced County, ECM services start July 1, 2022.
Community Supports can help you with housing, food, substance use disorders and more.

Who do I call if I have questions about Enhanced Care Management or Community Supports?
If you are an Alliance member, call the Alliance Member Services Department at 800-700-3874 from 8 a.m. to 5:30 p.m., Monday through Friday. If you need language assistance, we have a special telephone line to get an interpreter who speaks your language at no cost to you.
