Enhanced Care Management and Community Supports
Care coordination, housing assistance and other services for eligible members
The Alliance offers services to eligible members through two programs called Enhanced Care Management (ECM) and Community Supports. These services help people with complex health issues and people who need help setting up their medical care. They can help you get healthy and stay as healthy as possible.
ECM can help guide you with your medical care. ECM can also help you find community services that are related to your care.
Community Supports can help you with housing, getting food after a hospital discharge, hospital discharge support and more.
