Enhanced Care Management and Community Supports
Care coordination, housing assistance and other services
The Alliance offers services to eligible members through two programs called Enhanced Care Management (ECM) and Community Supports (CS). These services help people with complex health issues and people who need help setting up their medical care. They can help you get healthy and stay as healthy as possible.

Enhanced Care Management (ECM)
ECM can help guide you with your medical care. ECM can also help you find community services that are related to your care. ECM is available for both adults (ages 21 and over) and youth (ages 20 and under).
Community Supports (CS)
Community Supports can help you with housing, getting food after a hospital discharge, hospital discharge support and more.
If you are not sure if you qualify for ECM/CS services, contact the Alliance Member Services Department at 800-700-3874 from 8 a.m. to 5:30 p.m., Monday through Friday. If you need language assistance, we have a special telephone line to get an interpreter who speaks your language at no cost to you.
