As we embrace the new year and the changes it may bring, I appreciate that one thing remains the same: our longstanding commitment to collectively focus on the well-being of our community members.

Amidst the hardships many Alliance members face, such as housing, food security and transportation issues, our shared goal is to support them in being their healthiest selves. By acknowledging the realities our members grapple with, we can work together towards our vision of Healthy People, Healthy Communities.

Regular checkups play a crucial role in this journey, as they:

  • Ensure needed vaccines and other preventative care remains on track.
  • Provide an opportunity to discuss any potential or new health concerns.
  • Support overall well-being.
  • Ensure any chronic conditions remain in check.

When members schedule these checkups, they are taking a meaningful step towards improving their health and ultimately, improving their quality of life. Encourage community members you work with to schedule regular checkups as an essential component of a healthier life.

For community partners: If you are aware of members who need help getting to their appointments, scheduling their appointments, or if they need help with food, housing or other needs, please have them call Member Services at 800-700-3874.

For providers: If you have patients who have challenges getting into or keeping appointments, please refer them to our Care Coordination team at 800-700-3874, ext. 5512 or via fax at 831-430-5852
