Scheduling back-to-school checkups + online forms you should know about


New Provider Manual is now available

Please review the updated Provider Manual, effective April 1, 2024.

Provider role in back-to-school prep: Time to schedule those checkups!

Kids end up starting school late because they are unable to get required vaccines in time for the school year. The Alliance is encouraging parents to schedule back-to-school checkups far in advance.

Please join us in helping kids be school-ready and stay on track with their checkups and vaccines. You can use Provider Portal reports to identify patients 3-21 years old that are due for immunizations and well-child visits.

Spring checkups campaign

We are reaching out to members via channels including:

  • Digital flyers distributed at schools.
  • Radio ads.
  • Ads on buses and at the DMV.
  • Social media (Facebook, YouTube and Snapchat ads).

We have more detailed information for members on our Checkups page. Through our Healthy Start program, members can earn Target gift cards for completing their children’s checkups and vaccines on time.

Earn incentives

Don’t forget that primary care providers can earn Care-Based Incentives for several measures that track rates of child checkups, immunizations and screenings.

Care Management Referral Form is now digital

The Care Management Referral Form is now digital. Previously, provider offices were required to download and fill out a PDF and fax it back to us. We have now reduced the number of steps needed for submission; all you need to do is fill out the form online and click “Submit.”

These forms are secure and HIPAA-compliant. We will keep providers updated as other forms transition to digital submissions.
