Health Rewards Program
The Alliance’s Health Rewards Program rewards you and your family for taking actions that support your health!
We have rewards for getting routine care, managing chronic conditions, adopting healthy habits and more. Check out our rewards for all ages below!
Important information about all Alliance rewards
- You must be an Alliance member at the time of service to be eligible for the reward. Rewards are for members who have the Alliance as their only health care coverage. Those with other health coverage are not eligible.
- Gift cards cannot be used to buy firearms, alcohol or tobacco.
- Lost or stolen cards cannot be replaced.
- You must claim the Target gift card within 12 months of becoming eligible for the reward.

Reward amount: $25-$100 Target gift cards The Healthy Start program is for members ages 0-15 months, 15-30 months, 2-year-olds, 13-year-olds and 18 to 21-year-olds to get a healthy start on life and get rewarded! |
Baby Flu Vaccine Reward |

Reward amount: Chance to win $100 Target gift card Children who get the flu are at risk of health problems, especially if they are under the age of 2. A flu vaccine is the best way to protect your child from flu-related illness. |
Healthy Moms and Healthy Babies |

Reward amount: Chance to win $50 or $25 Target gift card The Healthy Moms and Healthy Babies (HMHB) program offers education and helps pregnant women get early prenatal and postpartum care. |
Healthy Weight for Life |

Reward amount: Target gift card for up to $100 Childhood obesity puts kids at risk for health problems. Healthy Weight for Life workshops teach parents how to help their children have a healthy weight and lifestyle. |
Healthier Living Program |
 |
Reward amount: Target gift card for up to $50 Living with a chronic condition like diabetes, depression or high blood pressure can be hard. Our 6-week Healthier Living Program will help you learn how to better manage your health. |
Live Better with Diabetes Program |
 |
Reward amount: Target gift card for up to $50 It can be hard to keep diabetes under control. Our 6-week Live Better with Diabetes Program Workshop will help you learn how to eat the right foods, be more active and lower stress. |
Nurse Advice Line |

Reward amount: Chance to win $50 Target gift card You can call the Nurse Advice Line anytime you have questions about your health or your child’s health. A registered nurse will help you with what to do next. |
