Collaboration is one of our core values at the Alliance. The annual Community Impact Report showcases our recent community investments and our ongoing presence within the communities we serve.
In 2023, the Alliance awarded $38 million to local organizations—a testament to our belief that unity is key for a healthier community. Last year, over 12,250 Alliance members were directly impacted by these grants, which provided critical care and social support to our communities. Our outreach program reached 14,500 members at in-person events, where Alliance staff connected directly with members.
In 2023, the Alliance:
- Attended 103 community events.
- Launched 8 new grant opportunities that fund a diverse array of committed community partners.
- Awarded $2.65 million in capital grant funding to help Community Bridges relocate and expand their Elderday Adult Day Health Care program at its new south Santa Cruz County facility.
Prioritizing community presence
Your Health Matters, the Alliance outreach team, reached more than 14,500 members in face-to-face interactions last year. YHM was at events that helped raise awareness for mental health, community safety, newborn and maternal health and more.
Investing in children and vulnerable communities
We understand that health care is comprehensive and extends beyond doctor appointments.
Last year, the Alliance used funding to:
- Recruit Community Health Workers who help provide individual support for Medi-Cal members and access health care.
- Work with local partners to reduce barriers to receiving on-time vaccinations.
- Increase behavioral health services on school campuses for students.
- Pilot street medicine services for members experiencing unsheltered homelessness.
