The Executive Summary offers a high-level overview of the priorities and goals we believe are the most important and impactful for the communities we serve. Over the next five years, we will focus our efforts on health equity and person-centered delivery system transformation.
To effect change and achieve better outcomes for the 390,000+ members we serve, your partnership is vital. We have so many shared goals for our communities and hope that the priorities we’ve outlined will be energizing and unifying as we move forward toward a vision of Healthy People, Healthy Communities.
