Medi-Cal Fee-for-Service changes start January 1, 2027

Starting January 1, 2027, some Alliance members will get their Medi-Cal care in a new way. They will move from the Alliance to Medi-Cal Fee-for-Service (FFS). This change is because of a new federal rule. It was not made by the Alliance.

If this change affects you, you are not losing your Medi-Cal. You will still have Medi-Cal, but the way you get your care will change.

Fee-for-Service does not mean you pay. Medi-Cal will still pay for your covered care.

Does this change affect me?

This change affects Medi-Cal members who are:

Undocumented.
Green card holders who must wait five years for full Medi-Cal.
Allowed to live in the U.S. but do not have official immigration status (PRUCOL).

If you are in the Program of All-Inclusive Care for the Elderly (PACE), you will stay in PACE and will not move to Medi-Cal Fee-for-Service.

What is Medi-Cal Fee-for-Service?

With Medi-Cal Fee-for-Service, you can get covered care from doctors and other providers that accepts Medi-Cal Fee-for-Service. You will choose your own doctor.

You can still get mental health care. Therapy, counseling and medicine visits will be covered through Medical Fee-for-Service. Care for serious mental health conditions and help with drug and alcohol use will still come from your county.

Your medicines will still be covered through Medi-Cal Rx. Your dental care will stay the same.

You will use your Medi-Cal Benefits Identification Card (BIC) when you get care. Bring your BIC card to your medical, dental and pharmacy visits. Your Alliance member ID card will no longer work after you move to Fee-for-Service.

Before January 1, 2027, ask your doctor or clinic if they accept Medi-Cal Fee-for-Service.

If they do, you can keep getting care there. If they do not, you will need to find a doctor or clinic that takes Medi-Cal Fee-for-Service.
