Community Supports: Medically Tailored Meals Form

Medically Tailored Meals (MTM) and Medically Supportive Food (MSF) are no-cost services for eligible Medi-Cal members with certain medical needs. These meals help members heal after a hospital stay or manage chronic conditions like diabetes or heart disease.

This form helps us see if you qualify. Submitting the form does not mean you will get these=[ services. Participation is your choice.
