Get extra help with Care Management
Basic care management
It can be tricky to keep track of your health care when you are seeing different doctors or getting care from different places. That is where we come in! The Alliance can help you get all the medical services, prescription medicines and behavioral health services you need, at no cost to you. We can help even if another program covers your services.
Here are some ways we can make things easier:
- After the hospital: Did you just get out of the hospital? We can help you set up those important follow-up appointments and make sure you get your medicines.
- Getting around: Do you need a ride to see the doctor? No problem! We can arrange free transportation for you. Learn how to ask for transportation services.
Complex care management
Some people have more serious health needs, and that is okay! If you are one of those people, you may be able to get extra help. The Alliance offers Complex Care Management (CCM) services to give you the support you need.
We can help you in the following situations:
- Your child is eligible for California Children’s Services (CCS) or is being evaluated for CCS-eligible conditions.
- You have conditions that need special attention like diabetes, asthma or obesity.
- You have gone to the hospital a lot of times.
- You need help checking your medicines.
- You have serious health challenges like a tough injury, HIV/AIDS, or are near the end of life.
- You have other health concerns like a seizure, a stroke, a wound that is not healing, or a condition like multiple sclerosis or Parkinson's disease.
