Provider Directory

The Provider Directory is the best way for Alliance members to find a primary care provider (PCP) in the Alliance network and near them. Members can also use the directory to find specialists, like gynecologists or pediatricians.

The directory has most of the information a member might need to know about a provider, including:

• Phone numbers.
• Office hours.
• Office location.
• Whether the provider is accepting new patients.
• Practitioner languages.

If members do not choose a PCP within 30 days, the Alliance will choose one for them.

To verify the availability of providers, members can call Alliance Member Services department at 800-700-3874 between 8 a.m. and 5:30 p.m., Monday through Friday.

Alliance members can find the directory online, download it as a PDF or the Alliance can mail members a printed Provider Directory. See below for more resources.

• Search the Online Provider Directory.
• Use the Provider Information Change Form to alert us to any changes to your provider information.
• Use the Provider Directory Information Attestation Form to attest to your current provider information.
