TotalCare (HMO D-SNP) What to do after an emergency room visit
We are glad you are on the road to recovery. Here is what you can do next to stay healthy and not have to go back to the emergency room (ER):
- Follow the instructions you got at the hospital. This is important because it helps you get better and keep small issues from getting big. If you are not sure what to do, ask your doctor or nurse. They will help you understand.
- Make an appointment with your doctor. Call your doctor’s office to make a follow-up appointment. This is your chance to talk about your ER visit and make a plan for the future. Your doctor’s name, address and phone number are listed on your Member ID card. If you are not sure who your doctor is, you can call TotalCare Member Services for help. The phone number is at the bottom of this page.
- Ask your doctor important questions. These questions will help you understand your treatment plan.
- What is my main problem?
- What do I need to do?
- Why is it important for me to do this?
- Keep your doctor informed: Your primary doctor is the health professional who should know you best. It may take time to feel comfortable with your doctor, but you can build trust by going to your checkups and communicating openly. Let them know if you have any new symptoms, or if your old symptoms return. Do not go back to the emergency room unless it is a life-threatening emergency.
- Call the Nurse Advice Line. Nurses are available 24/7 at 844-971-8907 (TTY: Dial 711) to answer your questions. If you cannot get in to see your doctor right away or have questions outside of office hours, the Nurse Advice Line can help. When you call the Nurse Advice Line, you will be entered into a monthly raffle to win a $50 Target gift card!
Feeling better after being in the hospital can take a while, and that is okay. Just remember – you are not alone.
