TotalCare Health and Wellness Programs
TotalCare (HMO D-SNP) is committed to improving access to affordable health care for our members.
TotalCare (HMO D-SNP) health and wellness programs are managed by a multidisciplinary team of experienced health educators. Health educators utilize motivational interviewing and trauma-informed care techniques to help members stay healthy and manage chronic disease.
Health program interventions follow evidence-based models/curriculums and/or a combination of best practices. Interventions can be done telephonically and via in-person workshops, virtual learning and member mailings.
To refer an TotalCare (HMO D-SNP) member to one of our programs, please complete the Health Education and Disease Management Programs referral form and fax it to 831-430-5852, attention: Health Programs. Do not include visit notes or lab results.

TotalCare (HMO D-SNP) Live Better with Diabetes (LBD) program is a series of self-management workshops designed to support adult members diagnosed with diabetes. This evidence-based program spans six weeks and helps participants manage their condition. To ensure accessibility, workshops are offered in multiple formats, including telephone calls, virtual classes and in-person classes. Members who participate in a six-week workshop will receive a Target gift card for up to $50.
TotalCare (HMO D-SNP) Healthier Living Program (HLP) is a series of workshops designed to support adult members diagnosed with chronic disease(s) such as asthma, cardiovascular disease, anxiety, depression or any other chronic disease(s). This evidence-based program spans six weeks and helps participants manage their condition. To ensure accessibility, workshops are offered in multiple formats, including telephone calls, virtual classes and in-person classes. Members who participate in a six-week workshop will receive a Target gift card for up to $50.
TotalCare (HMO D-SNP) can participate in weight management education services. This program provides access to and support with the WeightWatchers (WW) program.
TotalCare (HMO D-SNP) offers the Tobacco Cessation Support Program (TCSP) to help members quit smoking. TotalCare provides tobacco cessation benefits and services that support prevention and cessation of tobacco use.
