The Mobile Care Clinic travels throughout Merced County with a highly skilled health care team and a community health navigator.

- Sept. 10: Cultiva Central Valley/GEO Re-Entry, 9 a.m. to 3 p.m.
- Sept. 11: Flanagan Park, 9 a.m. to 3 p.m.
- Sept. 14: D Street Shelter, 9 a.m. to 3 p.m.
- Sept. 15: Merquin Elementary School, 9 a.m. to 3 p.m.
- Sept. 16: Sports Complex, 9 a.m. to 3 p.m.
- Sept. 18: Applegate Park, 9 a.m. to 3 p.m.
- Sept. 21: Rescue Mission Village of Hope, 9 a.m. to 3 p.m.
- Sept. 22: El Nido School, 9 a.m. to 3 p.m.
- Sept. 23: McNamara Park, 9 a.m. to 3 p.m.
- Sept. 24: Rancho San Miguel, 9 a.m. to 3 p.m.
- Sept. 25: Fahrens Park, 9 a.m. to 3 p.m.
- Sept. 28: Navigation Center, 9 a.m. to 3 p.m.

- Bilingual staff (English/Spanish).
- Evaluation and treatment of episodic medical conditions.
- Identification of medical homes for those patients with chronic needs.
- Identification of social services and resources in the community.
- Blood pressure screenings.
- BMI screenings.
- Glucose screenings.
- Social services referrals.
- Health insurance enrollment referrals.
- Community resources referrals.
