California Integrated Care Management (CICM)
CICM gives extra support to members who need help managing their care. Your care manager will work with you and your providers to create a care plan that matches your health goals and supports your medical, mental health and everyday needs.
This support can include help with:
- Making doctor appointments.
- Working with different providers to coordinate your care.
- Keeping track of your medicines.
- Getting mental health or social services.
- Finding and using local community resources
You may be automatically enrolled in CICM if you are part of one or more of these groups:
- You’ve gone to the emergency room or hospital many times.
- You don’t have stable housing or are experiencing homelessness.
- You have a serious mental illness (SMI) or substance use disorder (SUD).
- You need help at home to stay safe and out of a nursing home.
- You live in a nursing home and want to move back home.
- You are pregnant or just had a baby and need extra support.
- You have memory loss or dementia and need help at home.
- You are very sick and need help managing pain or other symptoms.
- You are transitioning from a correctional facility or were released from one within the past 12 months
Even if you are not automatically enrolled, you or your provider can ask to be evaluated for CICM at any time.
- Automatic enrollment: If you qualify, you may be signed up for CICM automatically. We will let you know if this happens.
- Referral-based enrollment: You, your caregiver or your doctor can ask us to see if CICM is right for you.
CICM is your choice. You can stop getting care management help at any time. If you stop using CICM, it won’t change your TotalCare plan or the benefits you get.
