Health Risk Assessment for TotalCare (HMO D-SNP) Members

If you are a TotalCare (HMO D-SNP) member, a Health Risk Assessment (HRA) is a tool that is part of your care management. It helps your doctor learn about your health conditions and how your daily habits affect your health.

Please fill out this form only if:

• You have not yet received or completed a TotalCare HRA.
• You need to complete your annual HRA update.
• You need to update your HRA because you have had a serious change in your health condition.

You can also fill out the HRA form by:

• Completing the form on our secure My Health Member Portal.
• Using your mobile phone. Text #HRA to 59849. Note: Members who have opted out of TotalCare texting must opt in again by texting START to 59849.
