The Mobile Care Clinic travels throughout Merced County with a highly skilled health care team and a community health navigator.
The upcoming dates and locations for the mobile care clinic include:
- August 7: Cultiva Central Valley, 9 a.m. to 3 p.m.
- August 14: D Street Shelter, 9 a.m. to 3 p.m.
- August 21: Salvation Army, 9 a.m. to 3 p.m.
- August 28: Applegate Park, 9 a.m. to 3 p.m.
The following services are available:
- Bilingual staff (English/Spanish).
- Evaluation and treatment of episodic medical conditions.
- Identification of medical homes for those patients with chronic needs.
- Identification of social services and resources in the community.
- Blood pressure screenings.
- BMI screenings.
- Glucose screenings.
- Social services referrals.
- Health insurance enrollment referrals.
- Community resources referrals.
