- June 8: D Street Shelter, 9 a.m. to 3 p.m.
- June 9: Washington School, 9 a.m. to 3 p.m.
- June 10: Houlihan Park, 9 a.m. to 3 p.m.
- June 11: Rancho San Miguel, 9 a.m. to 3 p.m.
- June 12: Flanagan Park, 9 a.m. to 3 p.m.
- June 15: Rescue Mission Village of Hope, 9 a.m. to 3 p.m.
- June 16: Merquin Elementary School, 9 a.m. to 3 p.m.
- June 17: Le Grand Park, 9 a.m. to 3 p.m.
- June 18: Cultiva Central Valley/Geo Reentry, 9 a.m. to 3 p.m.

- Evaluation and treatment of episodic medical conditions.
- Identification of medical homes for those patients with chronic needs.
- Identification of social services and resources in the community.
- Blood pressure screenings.
- BMI screenings.
- Glucose screenings.
- Social services referrals.

- Community resources referrals.
