- May 19: Merquin Elementary School, 9 a.m. to 3 p.m.
- May 20: Le Grand Park, 9 a.m. to 3 p.m.
- May 21: Salvation Army, 9 a.m. to 3 p.m.
- May 22: Applegate Park, 9 a.m. to 3 p.m.
- May 26: El Nido School, 9 a.m. to 3 p.m.
- May 27: Houlihan Park, 9 a.m. to 3 p.m.
- May 28: Rancho San Miguel, 9 a.m. to 3 p.m.
- May 29: Flanagan Park, 9 a.m. to 3 p.m.
The following services are available:
- Bilingual staff (English/Spanish).
- Evaluation and treatment of episodic medical conditions.
- Identification of medical homes for those patients with chronic needs.
- Identification of social services and resources in the community.
- Blood pressure screenings.
- BMI screenings.
- Glucose screenings.
- Social services referrals.
- Health insurance enrollment referrals.
- Community resources referrals.
