Supporting behavioral health and social connection: A spotlight on older adults

Social connection is an important part of healthy aging, but nearly 1 in 4 older adults experience social isolation (CDC). While the upcoming holiday season is often associated with feelings of joy and celebration, it can be challenging for older adults who may be processing major life changes due to retirement, loss of loved ones, mobility limitations, chronic conditions, transportation barriers or living alone.

Learn about behavioral health services for Medi-Cal and TotalCare (HMO D-SNP) members, and what you can do to help connect older adults to the support they need.

What providers can do

Connect
Ask about members’ support systems and barriers to social connection, such as transportation or food insecurity. Consider connecting members to the appropriate adult and aging resources on our Community Resources page.

Screen
Use validated screening tools to screen annually or as needed for depression. Find more information in our Depression Screening for Adolescents and Adults Tip Sheet.

Screen for other behavioral health concerns as needed.

Refer
Connect members to appropriate services and follow up. Use the Behavioral Health Provider Referral Form or call Alliance Care Management at 800-700-3874, ext. 5512. TotalCare and Medi-Cal both cover services such as therapy and treatment of mental health and substance use disorders. (Members can learn about mental health benefits in their Evidence of Coverage (Member Handbook).

Social Prescribing
Consider connecting members to support such as senior centers, exercise or wellness programs, social groups, libraries, faith-based organizations, volunteering, and other activities that foster purpose and belonging.

Complete the Medi-Cal Fee-for-Service Provider Participation Survey

Beginning January 1, 2027, Medi-Cal members identified by DHCS as having Unsatisfactory Immigration Status (UIS) will transition from Alliance managed care to State Medi-Cal Fee-for-Service (FFS). This is a federal policy change established by DHCS and was not initiated by the Alliance.

The transition does not end a member’s Medi-Cal eligibility but will change how affected members access and pay for services.

The Alliance is asking providers to complete a short survey about their participation in Medi-Cal FFS. Your response will help us understand provider availability and support continuity of care for affected members.

Please complete the survey as soon as possible, even if you do not plan to accept FFS members or are still evaluating participation.

To learn more about how to prepare for the FFS transition, visit our 2027 Medi-Cal Fee-For-Service Transition webpage.
