- May 6: Le Grand Park, 9 a.m. to 3 p.m.
- May 7: Rancho San Miguel, 9 a.m. to 3 p.m.
- May 8: Flanagan Park, 9 a.m. to 3 p.m.
- May 11: D Street Shelter, 9 a.m. to 3 p.m.
- May 12: Washington School, 9 a.m. to 3 p.m.
- May 13: Houlihan Park, 9 a.m. to 3 p.m.
- May 14: Cultiva Central Valley/Geo Reentry, 9 a.m. to 3 p.m.
- May 15, Fahrens Park, 9 a.m. to 3 p.m.
The following services are available:
- Bilingual staff (English/Spanish).
- Evaluation and treatment of episodic medical conditions.
- Identification of medical homes for those patients with chronic needs.
- Identification of social services and resources in the community.
- Blood pressure screenings.
- BMI screenings.
- Glucose screenings.
- Social services referrals.
- Health insurance enrollment referrals.
- Community resources referrals.
