Medication Therapy Management Program
What is the MTM program?
The Medication Therapy Management (MTM) Program is an additional service that is available at no cost to eligible TotalCare members with certain medical conditions. This program is run by MedImpact, on behalf of TotalCare. It helps you and your doctor make sure that your medications are working well. It also helps us find and fix any possible medication problems. Enrolling in the MTM program is your choice and does not affect Medicare drug coverage.
The program includes these benefits:
Targeted Medication Review (TMR)
At least quarterly, we review your prescription medications. If we find a possible issue, we will contact you and/or your doctor by fax, phone, or mail.
Comprehensive Medication Review (CMR)
At least once a year, you get a free 30-minute review of all your medications (including over-the-counter products). A pharmacist or other health professional will talk with you over the phone.
The CMR will:
- Be private and done by phone.
- Check for duplicate or unsafe medicines.
- Help you organize your medications and understand how and when to take them.
After the CMR, you’ll get a written summary that includes:
- A Recommended To-Do List with steps to help you get the best results.
- A Personal Medication List to help you keep track of your medications and know how to use them correctly.

Who can join the MTM Program?
If you qualify, you will be automatically enrolled. You’ll get a welcome letter within 60 days.
The letter will:
- Explain the program.
- Tell you how to opt out.
- Offer a chance to request a CMR.
You may also get a phone call offering the CMR. If you say yes, we will connect you with a qualified MTM provider to complete the review. You’ll also get information about safely throwing away controlled substances.
To take part in the MTM program, you must meet all of the following criteria:
- Take at least eight medicines covered by Medicare Part D.
- Have Medicare Part D medicine costs expected to be over $1,276 per year.
- Have a diagnosis of at least 3 of the following medical conditions:
- Alzheimer’s Disease.
- Bone disease-arthritis (including osteoporosis, osteoarthritis, and rheumatoid arthritis).
- Chronic congestive heart failure (CHF).
- Diabetes.
- Dyslipidemia.
- End-Stage Renal Disease (ESRD).
- Human immunodeficiency virus/Acquired immunodeficiency syndrome (HIV/AIDS).
- Hypertension.
- Mental health (including depression, schizophrenia, bipolar disorder, and other chronic/disabling mental health conditions).
- Respiratory Disease (including asthma, chronic obstructive pulmonary disease (COPD), and other chronic lung disorders).
