The Alliance is excited to offer TotalCare (HMO D-SNP). TotalCare is a Medicare Advantage Dual Special Needs Plan (D-SNP) for people 65 years and older, or adults 21 and older with certain disabilities, who are enrolled in Medicare and have Medi-Cal through the Alliance.
The Annual Enrollment Period runs through Dec. 7, 2025, with coverage effective Jan. 1, 2026.

TotalCare (HMO D-SNP) is a Medicare Advantage plan with a Medicare contract and a contract with the California Medicaid program. Enrollment in TotalCare depends on contract renewal.
