Understanding the connection between mental and physical health is essential to whole-person care. The Alliance values the contribution providers make in helping members live healthier lives.
Practical steps for everyday care
Providers can make a meaningful impact by:
- Encouraging routine screening for mental health, substance use and chronic conditions.
- Reinforcing healthy habits such as movement, nutrition, sleep and stress management.
- Coordinating care across behavioral health, primary care and community-based services.
- Reducing stigma by normalizing conversations about emotional wellbeing and recovery.
- Supporting staff wellness to strengthen resilience, engagement and quality of care.
Alliance programs and resources
Providers can support member wellbeing by connecting eligible individuals to Alliance programs. For members needing additional support, consider:
- Behavioral health referrals for assessment, treatment and ongoing support. Complete an online referral or leave a voicemail for Alliance Care Management at 800-700-3874, ext. 5512.
- Care management for comprehensive services to help members and their providers manage chronic or complex conditions.
- Health Education and Disease Management Programs for giving members the tools to get healthy and stay healthy.

2026 Data Submission Tool Guide now available
The 2026 Data Submission Tool (DST) Guide is now available in the Alliance Provider Portal. Get detailed instructions, screenshots, submission requirements and examples to support accurate and successful submissions.
Both the Data Submission Tool and the DST Guide can be accessed through the Alliance’s Provider Portal under the “Data Submissions” section. The DST supports providers in submitting supplemental data from electronic medical records and paper charts for Care-Based Incentive (CBI) and Healthcare Effectiveness Data and Information Set (HEDIS) measures.

- Breast Cancer Screening (screenings and mastectomies).
- Cervical Cancer Screening (Pap tests, HPV screenings and hysterectomies).
- Child and Adolescent Well-Care Visits (ages 0–21).
- Chlamydia Screening.
- Colorectal Cancer Screening.
- Controlling High Blood Pressure.
- Depression Screening and Follow-Up for Adolescents and Adults.
- Developmental Screening in the First 3 Years.
- Diabetic Poor Control (>9.0%).
- Application of Dental Fluoride Varnish.
- Immunizations for Children, Adolescents, and Adults.
- Initial Health Appointment (IHA).
- Lead Screening in Children.
- Post-Discharge Care.

- After you submit your data, you will receive an email confirmation within one business day.
- If your file was previously rejected, please review the reason for rejection, correct it and resubmit. If you have any questions about why the file was rejected, please contact your Provider Relations Representative.
