Do you know or work with adult Alliance Members who have diabetes or pre-diabetes? The Alliance offers the Live Better with Diabetes program (LBD) workshops for members 18 years and older who have diabetes or prediabetes. These workshops are offered in-person, virtually and telephonically.
We encourage you to print and share this flyer with Alliance members that you work with. The flyer is available in English, Spanish and Hmong, and has all the essential information members need to know to enroll.
Members will learn tools to keep diabetes under control, how to eat the right foods, be more active and lower stress.
Health Rewards Program
If members attend the full 6-week LBD workshop, they can receive a $50 Target gift card. If members attend 3-5 sessions of the workshop they can receive a $25 Target gift card instead.

You can have members call our Alliance Health Education Line at 800-700-3874, ext. 5580 to enroll. Our Health Educators are available Monday through Friday 8 a.m. to 5 p.m. You can also provide this useful online Health Programs Sign-up form for simple online enrollment which is available 24/7!
