TotalCare (HMO D-SNP) is a Medicare Advantage Dual Special Needs Plan (D-SNP) for people 65 years old and over and for some people with certain disabilities who are enrolled in both Medicare and Medi-Cal. The TotalCare plan coordinates your Medicare and Medi-Cal health benefits, all in one plan, to make it easier for you to get the health care you need. There’s no need to contact both Medicare and Medi-Cal if you have questions. Just call TotalCare and we’ll help you get care and answer your questions.
- Quality health care with $0 copays for doctor and hospital visits, no plan premiums or coverage gaps.
- Low- or no-cost prescription drugs.

- Extra benefits:
- Vision care. This includes one no-cost annual exam and $350 for glasses and lenses or contact lenses every 2 years.
- A flexible spending card. You will get a $100 allowance per quarter for over the counter (OTC) products.
- The Silver&Fit® Healthy Aging and Exercise program benefit with virtual and live services including digital workout plans and video library, home fitness kits, live 1:1 coaching, and access to participating fitness centers.
- Worldwide emergency coverage for emergency medical care outside of the U.S. ($50,000 maximum).

Your Care Coordinator can help you:
- Understand your health plan benefits.
- Find doctors and specialists within the network.

- Get Long-Term Services and Supports (LTSS) and help from community resources.
- Arrange transportation to and from your medical appointments.
- Schedule health screenings.
