Enhanced Care Management and Community Supports
Care coordination, housing assistance and other services
The Alliance has no-cost programs to help eligible Medi-Cal members navigate serious health needs and everyday challenges. These programs are:
- Enhanced Care Management (ECM).
- Community Supports (CS).
These services can help you stay healthy and safe by supporting your medical care, housing, food needs and daily living. If you need help finding a doctor, scheduling appointments or getting care, you may qualify for Enhanced Care Management. If you need help with housing, meals or support at home, you may qualify for Community Supports. You can apply for one or both, depending on your needs.
ECM is a no-cost Medi-Cal benefit that gives you a personal care team. Your care team helps you manage your health and connects you to services in your community. Adults (age 21 and over) and youth (age 20 and under) who meet eligibility criteria can get ECM services.
Your ECM care manager can help you:
- Find a doctor.
- Schedule appointments.
- Manage your medications.
- Connect you with housing, food, or mental health support.
- Help you after a hospital stay.
Community Supports can help you get housing, meals and support your daily needs. These services are for Medi-Cal members whose health is made worse by things like unstable housing or lack of food.
Community Supports can help you:
- Find or keep housing.
- Recover safely after a hospital visit.
- Get medically supportive meals.
- Make your home safer with accessibility upgrades like grab bars, ramps or widened doorways.
If you are not sure if you qualify for ECM/CS services, contact the Alliance Member Services Department at 800-700-3874 from 8 a.m. to 5:30 p.m., Monday through Friday. If you need language assistance, we have a special telephone line to get an interpreter who speaks your language at no cost to you.
