California Integrated Care Management (CICM)
CICM gives extra support to members who need help managing their care. You will work with a Care Manager and Care Team to make a care plan that matches your health goals and supports your medical, mental health, and everyday needs.
This support can include help with:
- Making doctor appointments.
- Working with different providers to coordinate your care.
- Keeping track of your medicines.
- Getting mental health or social services.
- Finding and using local community resources.
