Complex Care Workshop: Addressing Social Needs in Dementia Care

This free day-long workshop-style conference aims to empower health care teams to:
- Assess for cognitive impairment.
- Identify social needs of people with cognitive impairment.
- Learn skills and resources to manage their complex care.
The workshop will include sessions on how to conduct a cognitive health assessment and address common social needs of people with dementia and their caregivers, such as:
- Avoiding conservatorship.
- Advance care planning.
- Medi-Cal and long-term care.
Participants of the workshop are eligible to receive 3 CME/MOC and CE credits or up to 3 MCLE credits.
