About The Alliance

Central California Alliance for Health Website Privacy Policy

I. INTRODUCTION

We are very committed to protecting the privacy of your personal information (defined below), and the importance of security for all personal information is of utmost importance to Central California Alliance for Health (“the Alliance,” “we,” “us,” and “our”).

This privacy policy (“Privacy Policy”) is applicable to the Alliance website (the “Website”). By visiting and/or using the Website, you agree to this Privacy Policy. Your use of the Website is also subject to our Terms and Conditions. Any additional terms, conditions, or policies found on the Website that are more specific are controlling if they would otherwise be inconsistent with the Privacy Policy or the Terms and Conditions. If you do not agree to this Privacy Policy, please do not use the Website and exit immediately.

II. PERSONAL INFORMATION

Personal information is any information or data that is unique to an individual such as a name, social security number, address, e-mail address, birth date, etc. (“Personal Information”). We do not collect Personal Information about you unless you voluntarily submit such information.

Portions of the Website allow you to voluntarily provide us with Personal Information. For example, you may provide us with Personal Information to submit comments or grievances to us, to update your information, or to request materials or services from us.

We do not sell, license, transmit or disclose Personal Information that you provide to us outside of the Alliance except to contracted agents or vendors providing services supporting the Alliance operations or when required by law (please see Section 8 “Access by Third Parties” for additional information). Any and all such disclosure of any Personal Information is performed within the parameters of applicable laws and regulations.

III. ANONYMOUS INFORMATION

Anonymous information includes “traffic data,” “cookies” (described below) “IP Addresses” (described below), and other numeric codes that identify computers and non-Personal Information (“Anonymous Information”). We may use Anonymous Information to help us determine how people use parts of the Website so that we can improve the Website. We may also use or share Anonymous Information (or other information, other than Personal Information) in any other manner that we deem appropriate or necessary.

IV. COOKIES

One type of Anonymous Information is cookies, and we may use cookies to provide some of the Website services. “Cookies” are pieces of information that a website transfers to your computer’s hard disk for record-keeping purposes. The use of cookies is standard in the internet industry, and many major websites use them to provide useful features for their users.

The Alliance only leverages first-party cookies. First-party cookies are directly stored by your site visit. These cookies allow the Alliance to collect anonymous analytics data to maintain and improve user experience. The Alliance does not employ or allow third-party cookies of any type during your visit.

Cookies in and of themselves do not personally identify users, although they do identify a user’s computer. The Website never uses cookies to retrieve information from your computer that was not originally sent in a cookie. We do not use information transferred through cookies for any promotional or marketing purposes, nor is that information shared with any third parties whatsoever.

If you do not want or want to remove cookies, you may set your browser to refuse and/or delete them. This functionality is likely accessible in the web browser’s help file directions. Please note that refusing or deleting cookies may limit your ability to take full advantage of the Website.

V. INTERNET PROTOCOL ADDRESSES

Another type of Anonymous Information is an Internet Protocol Address (“IP Address”), which is a number that automatically identifies the computer or machine that you are using to access the Internet. The IP Address enables our server to send you the web pages that you want to visit. It may disclose the server owned by your Internet Service Provider. We may use your IP Address to help diagnose problems with our server and to support our site administration.

VI. SPYWARE

We never use or install spyware on your computer. We do not use spyware to retrieve information from your computer.

VII. PERSONS UNDER THE AGE OF 13

We do not knowingly collect any information from persons under the age of 13, without permission of the child’s parent or legal guardian. If you learn that your child has submitted Personal Information, and you would like to request that such information be removed from our system, please contact us by telephone at (831) 430-5500.

The Website is designed for adults. The Website is not intentionally targeted to children under the age of 13. Children under the age of 13 should not use the Website without obtaining prior parental consent.

VIII. ACCESS BY THIRD PARTIES

We occasionally have third-party agents, subcontractors, vendors, affiliates and joint ventures that perform functions on our behalf.  These entities may have access to the Personal Information needed to perform their functions and are contractually obligated to maintain the confidentiality and security of that Personal Information.  They are restricted from using or altering this data in any way other than to provide the requested services to the Website.

IX. LINKS TO OTHER WEBSITES

This Privacy Policy applies only to the Website, not to other websites, including other websites to which we may link. These links may include links from sponsors, partners, and others and are beyond our control. These other websites may send their own cookies to you, collect your data, or solicit your Personal Information. Always be aware of where you end up. We are not responsible for the actions and privacy policies of third parties and other websites. We encourage you to read the posted privacy statement and user terms and conditions whenever interacting with any other Website.

X. SECURITY POLICY

Personal Information collected by our Website is stored in secure operating environments that are not available to the public. We take reasonable steps to keep unauthorized users from accessing information through our computer network. We have appropriate security measures in place in our physical facilities to protect against the loss, misuse, or alteration of information that we have collected from you at the Website.

Only those employees who need access to your Personal Information in order to do their jobs are allowed access, each having signed confidentiality agreements. Any employee who violates our privacy and/or security policies is subject to disciplinary action, including possible termination and civil and/or criminal prosecution.

If you do not agree to the changes to the Privacy Policy, then please do not continue using our Website.
