Keep your Medi-Cal coverage in 2026

Make sure to renew your Medi-Cal on time!

If you renew on time, live in California and still meet Medi-Cal rules for income, you can keep your Medi-Cal coverage! Everyone must renew this year.

The Alliance is your Medi-Cal health plan if you live in Mariposa, Merced, Monterey, San Benito or Santa Cruz County. However, we do not process Medi-Cal applications or renewals. Your county Medi-Cal office will handle your Medi-Cal renewal.

3 steps to stay covered

If you've moved, update your contact information.

Make sure your county Medi-Cal office has your correct address and phone number.

Know your renewal date and watch your mail.

You should get a renewal letter mailed to you from your county Medi-Cal office with any steps you need to take to renew.

If you are required to complete renewal paperwork, fill it out and send it back fast.

Return your renewal to your county office by the deadline. If you do not renew your Medi-Cal before your renewal deadline, you will lose your benefits. To keep your coverage, complete your renewal each time it is due.

You can use the California Department of Health Care Services (DHCS) online tool to see whether you might qualify. It’s quick and easy! Note that you need to fill out a full Medi-Cal application to find out for sure.

What happens if I miss my renewal deadline?

If you do not renew your Medi-Cal, you will lose your Medi-Cal health coverage. You can still complete your renewal paperwork and submit to your enrollment agency to have your coverage reinstated. If you are late to renew your Medi-Cal, your coverage may change because of the new eligibility requirements. For example, you may only be able to get care for pregnancy or emergencies. Contact your county’s Medi-Cal office for help.

What is changing?

There are changes to Medi-Cal starting in 2026.

Most members will not see changes to their coverage. However, some members may see changes to their benefits and care.

Jan. 1, 2026

Enrollment freeze

You will not be able to sign up for full-scope Medi-Cal if you are ages 19 and older, undocumented and not pregnant. You can find more information about immigration status on the DHCS website.

If you had full-scope Medi-Cal on or before Dec. 31, 2025 and keep renewing your Medi-Cal on time, these changes will not affect your coverage this year.

Asset limit changes

Some people will no longer qualify for Medi-Cal benefits based on what they own (assets). This includes people whose Medi-Cal eligibility is based on:

  • Age.
  • Disability.
  • Long-term care needs.

The value of your assets will affect whether you are eligible for Medi-Cal. When you apply for or renew your Medi-Cal, the California Department of Health Care Services (DHCS) will ask you for information about the assets that you own.

  • An asset is a thing that you own that has value.
  • An asset limit is the dollar value of what you can own in countable assets and still qualify for Medi-Cal.

Dental coverage

You will not be eligible for dental benefits if you are 19 and older and have an unsatisfactory immigration status (UIS).
