Community Supports: Medically Tailored Meals Form

Medically Tailored Meals (MTM) and Medically Supportive Food (MSF) are no-cost services for eligible Medi-Cal members with certain medical needs. These meals help members heal after a hospital stay or manage chronic conditions like diabetes or heart disease.

This form helps us see if you qualify. Submitting the form does not mean you will get these services. Participation is your choice.
