If you have a baby, this infant wellness map can help you track your child’s development and visits to the doctor.

How to prepare for a well-child visit with the doctor

A well-child visit is when the doctor checks your baby’s growth and development. This is also a visit when your baby will get vaccinations.

Ask your doctor these 3 questions to better understand your baby’s health conditions and what you can do to keep him or her healthy:

  1. What is my baby’s main problem?
  2. What do I need to do?
  3. Why is it important for me to do this?

If you ever have any questions about your child’s development, ask your doctor. Trust your instincts — you know your child best!

How to track and improve your baby’s journey

Use this guide to know when to take your baby to the doctor from ages 0-12 months old.

Important Milestones: Your child’s first year

Birth

  • Recognizes caregiver’s voice.
  • Turns head toward breast or bottle.

1 month

  • Starts to smile.
  • Raises head when on tummy.

2 months

  • Turns head toward sounds.
  • Follows movement with eyes.
  • Starts to hold head up.
  • Begins to push up when on tummy.

4 months

  • Copies expressions like smiling and frowning.
  • Begins to babble.
  • Holds head steady by self, brings hands to mouth.

6 months

  • Knows familiar people from strangers.
  • Responds to own name, laughs or squeals with happiness.
  • Can roll over from tummy to back, bounces on legs when standing.
  • Shows curiosity, brings objects to mouth.

9 months

  • Makes lots of different sounds like “mamamama” and “babababa.”
  • Plays peek-a-boo.
  • Puts things in mouth, picks up things like cereal with thumb and first finger.
  • Can sit up, crawls, pulls to stand while holding on.

12 months

  • Cries when you leave, is nervous or shy with strangers.
  • Can do things like shake head no or wave goodbye, tries to say words you say.
  • Bangs two things together, follows directions like “pick up the toy.”
  • Gets into a sitting position without help, may stand by self.

  • RV.

4 months

Infant vaccine benefits

It is important that your child sees the doctor to get vaccines. Vaccines keep your baby healthy and prevent diseases.

These vaccines protect your child and reduce their risk of serious diseases.

Hepatitis B (HepB) Prevents liver disease and cancer.
Hepatitis A (HepA) Prevents fever, diarrhea, tiredness.
Diphtheria (DTaP) Prevents breathing problems, muscle spasms, whooping cough.
Hemophilic Influenza type B (Hib) Prevents meningitis (brain infection), pneumonia (lung infection), blood infection.
Pneumococcal (PCV) Prevents pneumonia (infection in the lungs), meningitis (brain infection).
Polio (IPV) Prevents muscle paralysis, disabilities, deformities.
Rotavirus (RV) Prevents severe diarrhea.
Chickenpox (VAR) Prevents blisters, skin infections, nerve damage, loss of eyesight.
Measles (MMR) Prevents pneumonia (infection in the lungs), deafness, brain damage.
Influenza (flu) Annual vaccine protects against serious illness caused by the flu (a respiratory virus).

If your child misses a vaccine, you don’t have to start over.

Call your doctor right away to schedule a “catch-up” visit or ask questions. It’s important that your child receives ALL vaccines.
