We recently identified that data submitted via the Data Submission Tool (DST) was missing in the August, September and October Quality Reports for the following measures:
- Diabetic HbA1c Poor Control.

- Depression Screening for Adolescents and Adults.
The issue for the November Quality Reports has been identified and fixed. We strongly recommend that you compare your November Quality Reports to your EHR for any possible gaps in care.
