Grantees with existing Capital Planning grants may still apply for Implementation grants in future application cycles based on their project timeline. Funding for Implementation grants is reserved for those entities with existing Planning grants; however, Implementation grants are not guaranteed and must be applied for separately.
COVID-19 Response Fund
The COVID-19 pandemic is disproportionately impacting the most vulnerable people in our community, many of whom are Alliance members. In recognition of the urgent and increasing COVID-19 related challenges facing Alliance members, the Alliance established a $1,000,000 COVID-19 Response Fund. Through this fund, the Alliance is quickly dispersing funds to non-profits providing aid to impacted individuals and families in Merced, Monterey and Santa Cruz counties. The COVID-19 Response Fund supports organizations addressing the urgent food security needs of Medi-Cal members. The fund also provides support to meet the basic health-related needs of our members in each county, such as hygiene and sanitation supplies.
Recuperative Care Pilot
Grants to recuperative care facilities to provide temporary housing with access to medical and other supportive services for Alliance Medi-Cal members experiencing homelessness and who are recovering from an acute illness or injury.
Grants to support health care and community-based organizations in implementing information technology and equipment systems that expand their capacity to serve the Medi-Cal population and result in improved access, coordination and quality of care.
Workforce Development Investments
Grants to support the development of new educational programs for licensed health care professionals that will serve the Medi-Cal population. (Not accepting new applications.
Technical Assistance Program
Grants for practice coaching or other consultant/training opportunities.
Intensive Case Management Program
Grants to support high-volume primary care practices in adding staff to provide intensive case management services for medically complex Medi-Cal patients within the patient-centered medical home (PCMH).
Post-Discharge Meal Delivery Program
Grants to fund the delivery of ready-made, nutritious meals to Medi-Cal members recovering from an inpatient hospital stay.
Practice Coaching Program
Grants to support practices in changing their clinic operations to improve quality, efficiency and patient experience through the adoption of the PCMH model of care
Grants to subsidize the cost of equipment that expands providers’ capacity to serve additional Medi-Cal members, improve practice efficiencies and improve member experience.
