Funding for Street Outreach Providers

Central California Alliance for Health is seeking partnerships with street outreach providers as part of our efforts for the Housing and Homelessness Incentive Program (HHIP).

HHIP is a Department of Health Care Services initiative that aims to expand and enhance programs that help people transition to community-based independent living arrangements. HHIP is a voluntary incentive program that will enable Medi-Cal managed care plans (MCPs) to earn incentive funds for making progress in addressing homelessness and housing insecurity as social determinants of health.

We are seeking to fund street outreach providers with the capabilities to:

• Implement outreach and engagement strategies focused on communities that are disproportionately impacted by homelessness in Merced and Monterey counties.
  • In Merced County, this includes people ages 50 and older; veterans; people identifying as black, indigenous or people of color; families with children; and unaccompanied youth (HHAP Round 3 application landscape analysis)
  • In Monterey County, this includes youth ages 18-24; adults with children; people identifying as Black, African American, Pacific Islander and Native American .
• Refer Alliance members that are experiencing homelessness to Enhanced Care Management and housing-related Community Supports.

Applicants must:

1. Have an active street outreach program.
2. Be a member of a local continuum of care program (Condado de Merced o Condado de Monterey) or plan to become a member.
3. Have the ability to initiate street outreach services in Merced or Monterey counties by no later than February 1,
4. Be able to follow street outreach best practices as outlined by the National Outreach Guidelines for Underserved Populations.

Upon review of submitted letters of intent, the Alliance may contact street outreach service providers that meet selection criteria to begin negotiations of Letters of Agreement to fund expansion of program capacity and infrastructure needs between January and October 2023.

Estimated funding per organization is $300,000. However, final funding amounts may be higher or lower based on the number and quality of responses received and the ability to expand identified services.
