The Mobile Care Clinic travels with a highly skilled health care team and a community health navigator to provide the following services:
- Bilingual staff (English/Spanish).
- Evaluation and treatment of episodic medical conditions.
- Identification of medical homes for those patients with chronic needs.
- Identification of social services and resources in the community.
- Blood pressure screenings.
- BMI screenings.
- Glucose screenings.
- Social services referrals.
- Health insurance enrollment referrals.
- Community resources referrals.
