Community Health Assessments and Community Health Improvement Plans

The Alliance collaborates with our local health jurisdiction (LHJ) partners on their Community Health Assessments (CHAs) and Community Health Improvement Plans (CHIPs).

Per APL 23-021, DHCS has modified the previous Population Needs Assessment requirements for Medi-Cal managed care plans (MCPs) to include a central requirement that MCPs collaborate meaningfully with local health jurisdictions (LHJs) on their current or next cycle of Community Health Assessments (CHAs)/Community Health Improvement Plans (CHIPs).
